Our referral form is provided by Jotform and our appointment scheduling by Calendly. Information you enter there is sent securely to Thrive Behavioral Therapy and used only to respond to you and to provide services. Please do not send health information by regular email, text or WhatsApp; use the referral form instead.

This website uses Google Analytics and the Meta pixel to understand how visitors use the site, using cookies and similar technologies. These tools are not used on our referral form. You can block cookies in your browser settings.
Health information
Protected health information you share with us as a client is handled according to HIPAA and our Notice of Privacy Practices, which we provide at intake.
